Anki add-on doesn't connect to browser extension, help!
Try switching where cards are sent to to MM. Then close the card creator window and open it again. That's what works for me when I get that issue

Generating Recommended Cards
I’ve run into the card creator getting stuck in the past as well. What usually works is editing/deleting something from the current card it’s trying to create (or removing that card from the queue) then it should get going again.

Is it impossible to learn German via Migaku?
1. Use the combine feature to combine adjacent subtitles but if there are multiple unknown words, set them to known or ignored before sending to the card creator or directly to anki
2. You can either do a one click (takes as long as it takes to record the audio) or send to the card creator and then send to anki - both cases can be a matter of a few seconds
3. I don't get what you mean but if it's only sending the translation to Anki then you haven't mapped the field correctly

Local books to Migaku
What do you mean by local books (physical or epub). Either way, you can use the legacy extension on brave browser so you can use the reader or convert the epub to a txt and paste that into the clipboard on the new extension. Or use NeatReader/TTSU to have it on a webpage that Migaku can read.
If it's a physical book, use the OCR feature on mobile

To answer why you've got different numbers, you need to specify which field to draw data from. If you're using just the back, then I'm assuming the sentence, target word and definition is being imported from your known cards

Why would it need to import the new words as unknown. If it parses a piece of media, anything in said media that you don't know is automatically unknown
